GEM Nurse

A GEM Nurse is a key member of the interdisciplinary team in the Geriatric ED. GEM Nurses will do targeted and focused assessment for older frail patients with the opportunity to identify intervene or optimize some of the geriatric syndromes with the overall goal to get them to right place at the right time. This role is crucial in identifying and co-ordinating the appropriate care and interventions for older people in the ED.
