Social Worker

Social workers have a very strong resources counselling focus and that is what they bring to the table as part of the interdisciplinary team in the senior-friendly ED. Social workers can ensure proper support for the patient and their family by connecting them with different useful resources in their community and helping them navigate the system. This will reduce the chance of a return to the ED, and improve long term outcomes for the patient.
